Ankle Fractures: A Complete Guide

Ankle fractures can involve the tibia, fibula or talus and range from a stable crack to a displaced break. The symptoms, how they are diagnosed and treated.

By Dr. Luigi ManziUpdated on

Ankle fractures can be partial or complete, affecting one or more bones in the foot. They are one of the most common injuries, both among athletes and those who engage in everyday activities. The severity of the injury can vary, but it is essential to have the foot examined immediately and treated appropriately to prevent complications and ensure a full recovery.

How does an ankle fracture occur? What are the most common symptoms of a broken ankle? What are the treatments? And in which cases is surgery necessary? In this guide, we'll try to answer the most frequently asked questions about ankle fractures.

Causes of ankle fractures

Ankle fractures

An ankle fracture occurs following an injury affecting the bones that make up the ankle joint. It's a serious injury that requires proper management and rehabilitation.

Not infrequently, a sprained ankle can lead to a fracture. Those who play sports like soccer or basketball, which involve jumping or rapid changes of direction and significant twisting, are particularly at risk.

Ankle fractures can be caused by high-impact trauma, in which a direct blow or sudden movement causes the bones to break, completely compromising the ankle's anatomy. These are the most serious and dangerous fractures, and can involve the tibia, fibula, and often the talus. In some cases, open fractures occur, in which the bone protrudes through the skin;

In older people, ankle fractures may be related to low-impact trauma. Osteoporosis, a condition in which bones become brittle, makes you more prone to fractures even from minor trauma.

Symptoms of ankle fractures

Doctors discuss

Symptoms of an ankle fracture include immediate, sharp pain, swelling, and difficulty moving the joint. Other symptoms of a fractured ankle include:

  • Visible deformity, in the case of complex fractures.
  • Difficulty walking or bearing weight on the ankle.
  • Ecchymosis (bruising) around the fracture area.

How can you rule out an ankle fracture? The presence of certain symptoms, such as swelling, deformity of the affected area, and difficulty walking, are undoubtedly clear signs of a possible ankle fracture. However, for a more definitive diagnosis, a discharge X-ray is necessary.

Treatments for ankle fractures

The diagnosis of ankle fractures is primarily clinical and should be made by an experienced orthopedic surgeon to determine the type of fracture and plan the most appropriate treatment. This depends on the severity of the fracture, which can be either compound (where the bone fragments remain in place) or displaced (where the bone segments lose contact and alignment).

Diagnosis through x-rays allows us to determine the type of fracture and, therefore, whether conservative or surgical treatment is necessary. If the ankle fracture is nondisplaced, immobilization with a cast or splint may be sufficient. Rest and ice are essential to reduce swelling and pain. Otherwise, surgery will be necessary to realign the bones and fix them with screws, plates, or wires to restore the alignment of the bone stumps. Surgery may also be necessary for multiple fractures or ligament damage.

Post-operative recovery can take several weeks to several months, depending on the severity of the fracture and the treatment received. Physical therapy is essential for regaining mobility and strength.

Ankle fractures require prompt and appropriate treatment to avoid long-term complications. Recognizing the symptoms and taking prompt action is essential for a full recovery. Appropriate treatment and good rehabilitation can allow the patient to return to normal daily activities.
